Confidentiality & Technology Usage Agreement

Purpose

It is essential that the business and internal affairs of CedarCreek Church be kept confidential.  As a volunteer or staff member of CedarCreek Church, it is expected that you will maintain the highest level of confidentiality in all matters concerning the church and any of its staff and/or attendees.

There may be times when you are exposed to confidential information and conversations that if made public could severely damage the reputation of CedarCreek Church and its attendees.

Your role makes accessing some of the information in our systems and databases necessary. We want to make sure we do all we can to allow access to the information needed while we safeguard the information entrusted to us. This agreement helps us to do that.  Please sign below and the form will be submitted to our team. Confidentiality

I understand that I am expected to use extreme caution when discussing any information about any attendees or staff of CedarCreek Church.  I understand that having been given access to CedarCreek Church’s systems, database, and/or other computer resources, I will be trusted to handle information that is sensitive and confidential. This includes all information about attendees, families, staff, and other associate organizations. As such, I will only access data as necessary to perform my duties as assigned.  I understand a family’s address and other personal information are considered private and must remain completely confidential. I will protect every individual’s privacy and use the database for church ministry purposes only, not my own personal use.

Account Information

I understand that my username, password, and any other electronic access credentials have been assigned to me exclusively for my use. In addition, I understand that supplying my credentials to another party constitutes a security risk to myself, CedarCreek Church, its computer network, its attendees, and its staff. As such, I agree to not share my username, password or any other access credentials with anyone, including my spouse or any other family member, and I will log out of systems when finished.

Ownership and Right of Access

I agree that CedarCreek Church shall retain ownership of any access credentials and resources that are assigned to me. I agree that CedarCreek Church reserves the right to monitor the usage of all CedarCreek Church-owned computers, software, and tablet activity, using any methods available to do so. Computers, equipment and/or any other devices belonging to the Church are not to be used to view, transmit, retrieve, or store any communications or images of a defamatory, discriminatory, harassing, or pornographic nature.

I am in agreement with and will follow the conditions set forth in the above statements. I also understand that violating any agreement stated above can result in the revocation of access privileges and/or other consequences as appropriate up to and including the immediate dismissal from my role.
